What is Gum Recession?

Gum recession is the loss of gum tissue along the gumline, which can make teeth look longer and expose the root surface. Recession can occur due to periodontal disease (gingivitis, periodontitis, advanced periodontitis), the natural aging process, thin gum tissue, or abrasive brushing habits.

Why Should the Gum Recession Be Taken Seriously?

When the gumline recedes, the tooth’s root surface may become exposed. Root exposure can increase the risk of:

  • Tooth sensitivity (especially to cold)
  • Root cavities and decay near the gumline
  • Gum inflammation and plaque accumulation
  • Ongoing recession and aesthetic concerns

Healthy gums are an important part of a healthy mouth—treating recession can help protect your teeth long-term and improve the appearance of your smile.

What is the Chao Pinhole Surgical Technique (PST™)?

The Chao Pinhole Surgical Technique™ (PST™) is a minimally invasive, incision-free approach to treating gum recession. Unlike traditional grafting, PST™ does not require taking donor tissue from the roof of the mouth.

How is PST™ Different From Traditional Gum Grafting?

Traditional gum grafting often involves placing donor tissue (or a soft tissue graft) over exposed roots and securing it with sutures. While grafting is an excellent option for many patients, it can involve:

  • A donor site (often the palate)
  • Sutures
  • More post-operative soreness and recovery time

PST™ is designed to reposition and stabilize existing gum tissue without scalpel incisions or sutures.

How is PST™ Performed?

During PST™, a small pinhole is created in the gum tissue using a needle. Through this access point, specialized instruments are used to gently loosen and reposition the gum tissue so it can be advanced over the exposed root surface.

No incisions are made, and the procedure focuses on adjusting the existing tissue in a careful, controlled way.

Benefits of the Chao Pinhole Surgical Technique (PST™)

Many patients choose PST™ because it may offer:

  • Less post-operative discomfort
  • Faster recovery compared to traditional grafting
  • No scalpel incisions and no sutures
  • No need to harvest tissue from the palate
  • Natural-looking, long-lasting root coverage in appropriate cases

*PST™ is also referred to as Chao Pinhole Gum Rejuvenation™ and the Lunchtime Gum Lift™.

Learn More About PST™

The Pinhole Surgical Technique (PST™) was invented and patented by John Chao, D.D.S. It is an incision-free procedure that uses a small pinhole and specialized instruments to reposition gum tissue over recession defects. Because there are no incisions, many patients experience minimal post-operative symptoms such as pain, swelling, and bleeding—and many are pleasantly surprised by the immediate cosmetic change.

Dr. Chao was granted a patent on the PST method and a separate patent on PST instruments by the U.S. Patent and Trademark Office.

Important Note: Every patient is unique. The best treatment option depends on the amount of recession, gum thickness, tooth position, bite forces, and overall periodontal health. A consultation and clinical exam are required to determine candidacy and expected outcomes.
